Social Service Assistant - County of Bergen Department of Human Services – Division of Disability Services
Under direction, performs paraprofessional work involved in the operations of a social service program Meals-on-Wheels; gathers and records significant social, financial, and pertinent information about their clients and/or implements selected aspects of the program; does other related duties as required.
Knowledge and Abilities:
- Interview clients and gather necessary program information.
- Conduct assessments and re-assessment to determine program eligibility.
- Coordinate meal services and deliveries.
- Maintain client data in database.
- Generate monthly reports.
- Provide case management and referrals to other social agencies in the community.
- Survey program participants on an annual basis to assess program satisfaction and effectiveness.
- Perform community outreach.
- Review and maintain program records and files.
- Contact and preserve cooperative relationships with other social agencies.
- Collaborate with social agencies to provide information and resources concerning nutrition.
- Maintain consistent communication with program participants.
- Assists in projects for the agency and gathers other routine data as needed.
- Responds to telephone inquiries regarding Meals-on-Wheels program in a timely manner and ensures information received is accurate.
- Fields incoming telephone calls from clients to suspend/resume service.
- Handles client comments and complaints calmly and politely; ensures that client issues are sufficiently resolved.
- Performs other tasks and duties as assigned by supervisor.

Education Requirements:
- Completion of at least 60 credits from an accredited college or university.
Other Requirements:
- Bilingual language skills a plus.
- Valid NJ Driver’s License.
- Experience and knowledge working with individuals with disabilities.
- Basic computer proficiency.
- Exemplary time management and organizational skills.
- Excellent verbal and written communication skills.
