Requirements

Completed Application. Departure date and Social Security number must be included.

Proof of American Citizenship (one of the following):

Birth Certificate: an original birth certificate, or a certified copy of your original birth certificate with a raised seal. PLEASE NOTE: a hospital certificate or baptismal certificate WILL NOT be accepted.

As a result of a joint U.S. Dept. of State and U.S. Dept. of Justice investigation Hudson County Clerk's Office of Vital Statistics and Jersey City issued birth certificates are no longer acceptable.  A NJ State issued Birth Certificate is required for application of a passport.  For a State issued replacement of a Hudson County Clerk and Jersey City issued birth certificate go to this link:

Please note: All other Hudson County town issued birth certificates are acceptable. 

Previous Passport

Naturalization Certificate: ORIGINAL ONLY. You will get a receipt for your certificate and will get it back with your passport.

Identification:

Driver's License: if you are over 17 (under 17 will use parent's license.)

If you do not have a driver's license: you must have 3 (three) forms of ID one of which must contain your signature such as: school/college ID, business/industrial ID, voter registration, credit cards.

Photographs:

Two (2) identical photos - 2" x 2" - color preferred. They MUST be with a white or off-white background. Photos are taken here at the passport office for $10.00.

Processing Time and Fees

Adult (16 & older):

REGULAR - $110.00 check made payable to US Department of State and a separate processing fee of $25.00 payable in cash. Customer will receive their passport within 6-8 weeks.

EXPEDITE - $184.96 check made payable to US Department of State and a separate processing &  fee of $39.96 payable in cash. Customer will receive their passport within approximately 3 weeks.

 

 

Child (15 & under):

REGULAR - $80.00 check made payable to US Department of State and a separate processing fee of $25.00 payable in cash. Customer will receive their passport within 6-8 weeks.

EXPEDITE - $154.96 check made payable to US Department of State and a separate processing & fee of $39.96 payable in cash. Customer will receive their passport within approximately 3 weeks.

COURIER SERVICE - We offer a messenger service to Philadelphia for emergency passports, if you are traveling in less than 7 days and already have your travel ticket.  The total cost is $300.00, in cash, per person, including the passport photos.  Because we are limited to the number of passport applications we are able to send, this service must be offered on a first come first serve basis. NATIONAL PASSPORT INFORMATION CENTER (NPIC) EXPANDED SERVICE

As of August 19, 2003, the National Passport Information Center (NPIC), the Department's single, centralized public contact center for U.S. passport information, will move from 1-900 fee-for-service to toll-free service.  This change is the result of a new contract with AT&T that will greatly expand the public's access to passport information.  The new, general telephone number will be 1-877-4USA-PPT (1-877-487-2778).  For TDD/ TTY users, the number will be 1-888-874-7793.  With this change, live operator service will also be expanded.  Live operators will be available on weekdays from 8:00am to 8:00pm, Eastern Time.  Twenty-four hour, 7-day access to automated information will continue to be available. 

Another significant improvement will be the addition of e-mail access to the National Passport Information Center at npic@state.gov. A response will be sent to e-mail inquiries within 24-hours during normal business days.  This service expansion compliments the vast amount of passport and other international travel information readily available on the Department of State's Bureau of Consular Affairs website, http://travel.state.gov

U.S. PASSPORT CARD APPLICATIONS ACCEPTED BEGINNING FEBRUARY 1

U.S. citizens may begin applying in advance for the new U.S. Passport Card beginning February 1, 2008, in anticipation of land border travel document requirements.  We expect cards will be available and mailed to applicants in spring 2008. 

The passport card will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.  The card may not be used to travel by air.   It will otherwise carry the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards.

The Department of State is issuing this passport card in response to the needs of border resident communities for a less expensive and more portable alternative to the traditional passport book.  The card will have the same validity period as a passport book: 10 years for an adult, five for children 15 and younger.   For adults who already have a passport book, they may apply for the card as a passport renewal and pay only $20.  First-time applicants will pay $45 for adult cards and $35 for children.

*Special Notice Regarding Passport Issuance to Children*

Effective immediately, the Federal Government is requiring that all applicants, including infants and children of all ages and adults who are not eligible to renew their passports by mail, must appear in person, whether you are applying for a new or renewal passport.  In effect since July 2, 2001, passport issuance for children age 15 and younger will require either the presence of both parents or written documentation from the parent who cannot appear in person giving his/her consent for the issuance of the child's passport.  This statement must contain the child's full name, date of birth, place of birth and give authorization to the applying parent to apply for the child's passport in his/her absence. This statement must be NOTARIZED and you will need a separate statement for each child.  Passport form DS-3053 (Statement of Consent) may be used in place of the written statement and must also be NOTARIZED. Additionally, birth certificates showing the names of both parents must be presented for all children age 15 and younger even when there was a previous passport issued. (Public Law 106-113, Section 236)
