Fees and Requirements for Opening a Business

	$50.00 to register a Trade Name.
	$52.00 to register a Trade Name and have it notarized here.
	Must have a business name that is not presently being used by any other business.
	Fill out four (4) Trade Name forms and have them Notarized.
	You will receive two (2) copies back, one marked "filed" for your own records and a "certified copy" for the bank to open a checking account in your business name.

Adding a New Owner

	$50.00 to Amend a Trade Name.
	$52.00 to Amend a Trade Name and have it notarized here.
	Fill out four (4) Trade Name forms and have them Notarized.
	You will receive two (2) copies back.

Closing a Business

	$25.00 to Discontinue a Trade Name.
	$27.00 to Discontinue a Trade Name and have it notarized here.
	Fill out four (4) Discontinuance forms and have them Notarized.
	You will receive two (2) copies back, one marked "filed" for your own records and a "certified copy" for the bank to close the checking account in your business name.

Taking Owners off the Business

	$25.00 for a Dissolution of a Trade Name.
	$27.00 for a Dissolution of a Trade Name and have it notarized here.
	Fill out four (4) Dissolution of Trade Name forms and have them Notarized.

Non-Profit / Church Corp. Filing

	$25.00 for filing
	Original & two (2) copies of Trade Name for non-profit church. All officers must sign.

Business Lists for Sale

	$40.00 for monthly detailed list of Trade Names.
	$10.00 for weekly detailed list of Trade Names.
	$400.00 for annual detailed list of Trade Names.
	Detailed list includes: name and address of business, name and address of owners, file date and file # and type of business.
